For over twenty-six years, Project Transition (PT) has provided help and hope to adults with psychiatric problems that have been recurrent and persistent. Many of the people we work with have felt failed by previous treatment efforts, including psychiatric hospitalizations, medications, and drug & alcohol rehabilitation programs.

Project Transition’s apartment-based therapeutic communities are supportive and caring. We promote sustainable recovery:

• the treatment and self-management of psychiatric difficulties (with or without coexisting substance abuse)
• the restoration of functioning, relationships, and self-direction
• reintegration into the community.
